• This form is used to drop below 12 credit hours after the Census Date, to add, drop, or modify a class after the drop deadline, and to gain permission to enroll in a class past the second attempt.
• Please note:
  • An academic advisor should sign the form no matter what
  • In certain circumstances (as specified on the form), the linked Supplemental Data Form is also required.
  • Once the form is completed please send it Myisshia Baldwin, mlbaldw2@ncsu.edu, who will pass it along to the dean’s office
